O objetivo deste estudo foi analisar a atenção aos doentes de hanseníase no sistema de saúde de Londrina,PR. Trata-se de estudo epidemiológico descritivo de corte transversal. Foram entrevistados 119 doentes de hanseníase, notificados no Sistema de Informações de Agravos de Notificação, no período de 2009 a 2012, e residentes na área urbana. Para a coleta dos dados, utilizou-se um instrumento adaptado para atenção à hanseníase - Primary Care Assessment Tool. A análise dos dados se deu por meio da análise univariada com cálculo de medidas de posição, de dispersão, de frequências e proporção relativa; análise bivariada, com a aplicação do teste qui-quadrado de proporções para as variáveis categóricas, e análise de variância (ANOVA) para as variáveis métricas, e quando violados os critérios dessa, recorreu-se ao teste de Kruskal-Wallis. Para análise multivariada, utilizou-se a análise de correspondência múltipla. Fixou-se o erro tipo 1 (?=0,05). Dos 119 entrevistados, 60 (50,42%) doentes eram do sexo masculino, 64 (53,78%) com idade média entre 42 e 65 anos, 79 (66,39%) possuíam ensino fundamental, 60 (50,42%) declararam-se casados, 56 (47,07%) referiram atividade econômica não formal, 107 (89,92%) residiam em domicílio de alvenaria, 69 (57,98%) residiam com mais de 3 pessoas. Os resultados apontaram que os doentes de hanseníase procuram as unidades mais próximas de sua residência, mas não é onde ocorre o diagnóstico e o tratamento. Os atributos concernentes ao diagnóstico e ao tratamento foram classificados respectivamente em não satisfatório e regular; o vínculo alcançou níveis satisfatórios. Os demais atributos, elenco de serviços, coordenação, participação da família e orientação à comunidade, não alcançaram valores satisfatórios. Verificou-se, ainda que aqueles com maior renda avaliaram mais insatisfatoriamente o acesso à atenção do que os de menor renda. Os resultados despertam reflexões quanto às políticas de descentralização das ações de eliminação da hanseníase

The aim of this study was to analyze the attention to the leprosy patients in the healthcare system of Londrina,PR. This is a descriptive, epidemiological and cross-sectional study. It was possible to interview 119 leprosy patients, who were reported in the Notifiable Diseases Information System, during in the period 2009/ 2012, and living in the urban area. For data collection, it was used an instrument adapted to the attention of the leprosy: The Primary Care Assessment Tool. Data analysis was performed by means of univariate analysis with the calculation of position measurements, dispersion, frequency and relative proportion. Also, it was possible to perform a bivariate analysis with the application of the chi-squared test of proportions to the categorical variables, analysis of variance (ANOVA) for metric variables, and when their criteria were violated, it was used the Kruskal-Wallis test. For multivariate analysis, it was used a multiple correspondence analysis. It was possible to fix the type 1 error in (?=0,05). Out of the 119 respondents, 60 (50.42%) were males; 64 (53.78%) had an average age between 42 and 65 years old, and 79 (66.39%) had primary education. Sixty of them (50.42%) reported being married; 56 (47.07%) reported an informal economic activity; 107 (89.92%) were living in masonry homes, and 69 (57.98%) were living with more than 3 people. The results showed that patients with leprosy look for the closest units to their residences; however, diagnosis and treatment occur in other places. The attributes concerning the diagnosis and treatment were ranked respectively as unsatisfactory and regular, and the connection achieved satisfactory levels. The remaining attributes, services available, coordination, family involvement and community orientation did not reach satisfactory values. It was also possible to verify that people with higher incomes evaluated more poorly the care access than people with lower income. The results arose reflections about policies of actions decentralization in order to eliminate the leprosy
