If you are a teacher that presented your habilitation theses at USP, you can make it fully available in the Digital Libray of Theses and Dissertations:

1. Acquire the form Formulário de Autorização (see how to fill it according to the model).
2. Fill the form, print it and sign it.
3. Acquire the form Formulário de Dados da Tese de Livre Docência.
4. Fill the form and print it.
5. Deliver personally or by mail the forms 'Formulário de Autorização' and 'Formulário de Dados da Tese de Livre Docência' and the media (CD-ROM, Floppy Disk, ZIP Disk) with the thesis' files to the library where you defended it.
6. In a matter of few weeks, your thesis will be available in the Digital Library.
