Com o envelhecimento populacional, passa a ser um desafio a assistência aos idosos considerando a complexidade das questões de saúde e as ações de atenção integral a partir da perspectiva funcional, nos moldes de linhas de cuidado ao idoso. Um dos métodos para avaliar e classificar o risco de declínio funcional é através da avaliação multidimensional. Na rede pública de saúde do município de São Paulo/SP, atualmente utiliza-se a Avaliação Multidimensional da Pessoa Idosa na Atenção Básica (AMPI/AB), instrumento que ainda não passou por processo de validação. Desse modo, a análise das propriedades psicométricas deste instrumento contribuirá na perspectiva de validação futura, uma vez que o desenvolvimento de instrumentos de medida no meio científico e clínico só terá reconhecimento após avaliar as propriedades psicométricas do mesmo. Sendo assim, o presente estudo tem como objetivo analisar as características psicométricas da AMPI/AB, a partir da coleta de dados do questionário inicial da AMPI/AB de 535 idosos, com idade média de 76,2 ±8,0 anos, sendo, 77,6% mulheres, assistidos por 10 equipes do Programa Acompanhante de Idosos, pertencentes à Coordenadoria Regional de Saúde Sul do município de São Paulo. Para a inclusão no estudo, foram considerados os idosos cujas AMPI/AB estivessem completas, aplicadas por profissional capacitado há menos de um ano e tivessem sido respondidas sem a interferência de terceiros. A AMPI/AB possui 31 itens individuais e 7 itens compostos distribuídos em 17 parâmetros: idade autorreferida, autoavaliação de saúde, arranjo familiar, medicamentos, internações, quedas, visão, audição, limitação física, cognição, humor, atividades básicas de vida diária, atividades instrumentais de vida diária, incontinência, perda de peso não intencional e condições bucais. Para a análise psicométrica utilizou-se a Análise Fatorial Exploratória (AFE), sendo considerado adequados valores de variância explicada 60%, cargas fatoriais 0,50 a 0,70, comunalidades 0,40 e correlações entre -1 e 1 e confiabilidade pelos índices de alfa de Cronbach e Ômega de McDonald 0,70 a 0,80, respectivamente. A primeira testagem de dimensionalidade, com modelo inicial de 38 itens resultou em 6 domínios com variância explicada de 16,3%, baixos valores de comunalidades e 13 itens com cargas fatoriais inferiores a 0,30. A confiabilidade do modelo inicial apresentou boa confiabilidade, acima de 0,85 para todos os índices. Na reanálise, para o modelo modificado foram retirados os itens com pior desempenho, resultando em um instrumento com 20 itens com cargas fatoriais 0,30 (exceto por dois itens), 5 dimensões, apresentando variância explicada de 8,3% e boa confiabilidade, com valores acima de 0,78 para todos os índices. A melhor correlação foi de 0,50, entre as atividades básicas/limitação funcional e instrumentais de vida diária/mobilidade. A análise psicométrica mostrou que a versão original da AMPI/AB não apresenta validade de construto aceitável. A versão modificada com 20 itens mostrou melhor adequação às técnicas utilizadas, mas ainda necessita de alterações para que a validade de construto seja confirmada. Para tanto, é necessário que seja definido o construto latente da AMPI/AB, com consequente ajuste do seu conteúdo, levando em consideração a literatura gerontológica, a padronização no tipo de resposta e a redação dos itens

With the aging of the population, it becomes a challenge for the assistance of the older adults considering the complexity of health issues and the actions of integral attention from the functional perspective, along the lines of the older adults care. One of the methods for assessing and classifying the risk of functional decline is through multidimensional evaluation. The municipality of São Paulos public health, is currently using the Multidimensional Evaluation of Multidimensional Evaluation of Older People in Primary Care (AMPI/AB), an instrument that has not yet been through the validation process. Thereby, the analysis of the psychometric properties of this instrument will contribute to the perspective of future validation, as the development of instruments of measurement in the scientific and clinical environment will only be recognized after evaluating the psychometric properties of it. Therefore, the present study aims to analyze the psychometric characteristics of the AMPI/AB, from the data collection of the initial AMPI/AB questionnaire of 535 older adults, with an average age of 76.2 ± 8.0 years, 77.6% were women, assisted by 10 teams of the Elderly Companion Program, belonging to the South Regional Coordination of the municipality of São Paulos Health Department. For inclusion in the study, were considered older people whose AMPI/AB were complete, applied by trained professionals in less than one year and were answered without third part interference. The AMPI/AB has 31 single and 7 composed items divided in to 17 parameters: self-rated age, health self-evaluation, family arrangement, medications, hospital admissions, falls, vision, hearing, physical limitation, cognition, mood, daily life basic activities, daily life instrumental activities, incontinence, unintentional weight loss and oral conditions. For the psychometric analysis it was used the Exploratory Factor Analysis (AFE), being considered adequate values of explained variance 60%, factorial loads 0.50 to 0.70, commonalities 0.40 and correlations between -1 and 1 and reliability by the Cronbach alpha and McDonald Omega indexes of 0.70 to 0.80, respectively. The first dimensionality test with an initial model of 38 items resulted in 6 domains with explained variance of 16.3%, low values of commonalities and 13 items with factorial loads lower than 0.30. The reliability of the initial model presented good reliability, above 0.85 for all indices. In the reanalysis, for the modified model, the worst performing items were discarded, resulting in an instrument with 20 items with factorial loads 0.30 (except for two items), 5 dimensions, with explained variance of 8.3% and good reliability, with values above 0.78 for all indexes. The best correlation was 0.50, between basic activities/functional limitation and instrumental of daily life/mobility. The psychometric analysis showed that the original version of the AMPI/AB with 38 items does not have an acceptable construct validity. The modified version with 20 items showed better adaptation to the techniques used, but still needs changes so that the construct validity is confirmed. For this, it is necessary to define the latent AMPI/AB construct, with consequent adjustment of its content, considering the gerontological literature, the standardization in the type of response and the writing of the items
