Diabetes Mellitus tipo 2 (DM2) é o tipo mais comum de diabetes, representando cerca de 90% de todos os casos diagnosticados. Em 2017 no Brasil, De acordo com a Federação Internacional de Diabetes (IDF) a prevalência de DM2 para a população brasileira era de 8 a 9% de indivíduos com mais de 18 anos. O DM2 confere um maior risco para doença cardiovascular (DCV). Os pacientes com DM2 descontrolados são propensos a infecções por fungos e bactérias em virtude dos efeitos adversos da hiperglicemia. As infecções por fungos no trato urinário afetam principalmente bexiga e os rins. A pesquisa dos artigos aconteceu na base de dados (PubMed/Medline), ClinicalTrials.gov, Cochrane e Scopus com os seguintes termos: "Diabetes Mellitus" and "Sodium Glucose Cotransporter 2". Na pesquisa foram incluídos artigos originais publicados em inglês, espanhol e português até novembro de 2018, sem restrição de idioma, todos em seres humanos, que tiveram como objeto de estudo o uso dos Inibidores do co-transportador sódio/glicose tipo 2 (iSGLT2) no tratamento do DM2 em pacientes que tenham recebido terapia padrão prévia. Este trabalho selecionou na varredura 4 trabalhos, num total de 19.572 pacientes, em que consideramos os seguintes desfechos: Infecção do Trato Urinário (ITU) e Fratura Óssea. Os iSGLT2 não apresentaram relevância estatística para ITU (Risco Relativo 0,99 [IC95% 0,93- 1,04]; p=0,76), entretanto aumentaram o risco de Fratura Óssea em 9,4% (Risco Relativo 1,094 [IC95% 1,005 - 1,192]; p=0,03). A classe dos iSGLT2 mudou consideravelmente a perspectiva no tratamento do DM2, seja no âmbito de eficácia e redução de desfechos cardiovasculares maiores, seja no quesito de segurança. Mesmo que esta metanálise tenha observado perfil de segurança satisfatório através dos desfechos de Infecção do Trato Urinário (ITU) e aumento do risco de fratura (9,4%), é preciso ter cautela em avaliar outros desfechos. No entanto, as amplas evidências de eficácia e segurança abordadas neste trabalho, dão respaldo aos gestores de saúde a incorporarem esta tecnologia nos serviços de atenção terciária, propondo uma revisão do atual protocolo de tratamento para pacientes com DM2 do serviço de atenção terciária à saúde.

Type 2 Diabetes Mellitus (DM2) is the most common type of diabetes, accounting for about 90% of all diagnosed cases. In Brazil (2017), according to the International Diabetes Federation (IDF) the prevalence of DM2 for the Brazilian population was 8 to 9% of individuals older than 18 years. DM2 confers a higher risk for cardiovascular disease (CVD. Patients with uncontrolled DM2 are more susceptibles to fungal and bacterial infections due to the adverse effects of hyperglycemia. Fungus infections in the urinary tract mainly affect the bladder and kidneys. This article has selected studies in the database (PubMed / Medline), ClinicalTrials.gov, Cochrane and Scopus with the following terms: "Diabetes Mellitus" and "Sodium Glucose Cotransporter 2". The study included original articles published in English, Spanish and Portuguese until November 2018, without restriction of language, all in humans, whose purpose was to study the inhibitors of co-transporter sodium / glucose type 2 (iSGLT2) in the treatment of DM2 in patients who have received previous standard therapy. This study selected 4 papers, in a total of 19,572 publishies, in whom we considered the following outcomes: Urinary Tract Infection (UTI) and Bone Fracture. The iSGLT2 did not present statistical relevance for ITU (Relative Risk 0.99 [95% CI 0.93-1.04], p = 0.76). Otherwise, it increased the risk of Bone Fracture in 9.4% (Relative Risk 1,094 [95% CI 1,005 - 1,192], p = 0,03). The iSGLT2 class has greatly changed the outlook for the treatment of DM2, both in terms of efficacy and reduction of major cardiovascular outcomes, and in terms of safety. Even if this meta-analysis has observed a satisfactory safety profile through the Urinary Tract Infection (UTI). Altough the outcomes for fracture risk has increased (9.4%). Therefore caution must be taken in evaluating other outcomes. However, the ample evidence of efficacy and safety addressed in this paper supports health care managers in adoptting this therapy into health care services, proposing a review of the current treatment protocol for DM2 patients.
