A Mielopatia cervical espondilótica é causa primária de disfunção da medula espinal em indivíduos adultos, e consequentemente é responsável por boa parte dos procedimentos cirúrgicos para tratamento das doenças da coluna vertebral, no entanto ainda restam dúvidas sobre a melhor via de acesso cirúrgica. O objetivo deste estudo foi avaliar as complicações pós-operatórias precoces (ocorridas até 30 dias após a cirurgia) associadas à abordagem cirúrgica da coluna cervical em pacientes portadores de mielopatia cervical espondilótica (MCE), comparando a abordagem anterior, a abordagem posterior e a abordagem combinada. Trata-se de um estudo retrospectivo de um banco de dados, sendo que 169 pacientes preencheram os critérios de inclusão. Foram avaliados dados demográficos, como gênero e idade, e dados cirúrgicos, como a via de acesso realizada, número de segmentos artrodesados, tempo cirúrgico e as complicações. As complicações foram divididas em maiores (infecção profunda da ferida operatória, intercorrência com o implante, nova compressão precoce, insuficiência cardíaca) e menores (disfagia, infecção superficial, dor, intercorrência urinária, neuropraxia da raiz de C5, estado confusional agudo, hematoma ferida operatória). Foram incluídos 169 pacientes, sendo 57 do sexo feminino (33,7%) e 112 do sexo masculino (66,2%). A idade variou de 21 a 87 anos, média de 56,48 anos (+/- 11). Destes, 52 (30,8%) foram submetidos a abordagem anterior, 111 (65,7%) à abordagem posterior e 6 (3,5%) à abordagem combinada (anterior e posterior). Assim como, na literatura prévia, evidenciamos a disfagia, a dor e a infecção superficial da ferida operatória, como as complicações pós-operatórias mais frequentes. No entanto, não foi possível estabelecer uma relação estatística entre a incidência de complicações e o tempo cirúrgico, a via de acesso e o número de segmentos fixados.

Cervical spondylotic myelopathy is the primary cause of spinal cord dysfunction in adult individuals, and consequently is responsible for most surgical procedures for the treatment of spinal diseases, however, there are still doubts about the best surgical approach. The aim of this study was to evaluate early postoperative complications (occurring up to 30 days after surgery) associated with the surgical approach to the cervical spine in patients with cervical spondylotic myelopathy (CSM), comparing the anterior approach, the posterior approach and the approach combined. This is a retrospective database study, with 169 patients fulfilling the inclusion criteria. Demographic data, such as gender and age, and surgical data, such as the surgical approach performed, number of arthrodesis segments, surgical time and complications, were evaluated. Complications were divided into major (deep surgical wound infection, implant complications, early re-compression, heart failure) and minor (dysphagia, superficial infection, pain, urinary complications, C5 root neuropraxia, acute confusional state, wound hematoma operative). A total of 169 patients were included, 57 females (33.7%) and 112 males (66.2%). Age ranged from 21 to 87 years, mean of 56.48 years (+/- 11). Of these, 52 (30.8%) underwent an anterior approach, 111 (65.7%) a posterior approach and 6 (3.5%) a combined approach (anterior and posterior). As in previous literature, we evidenced dysphagia, pain and superficial surgical wound infection as the most frequent postoperative complications. However, it was not possible to establish a statistical relationship between the incidence of complications and the surgical time, the surgical approach and the number of fixed segments.
