A passagem de plantão, ato de transferir informações e responsabilidade entre os médicos, é reconhecida como um dos elementos chave para qualidade do serviço prestado e segurança do paciente. Em 2014, foi publicado um estudo multicêntrico de intervenção que resultou em um mnemônico, o I-PASS (Illness severity, Patient summary, Action list, Situation awareness and contingency planning, Synthesis by receiver), cujo uso padronizado resultou em diminuição em 23% nos erros médicos e em 30% nos eventos adversos evitáveis em uma população pediátrica. Esse estudo objetivou implantar esse sistema padronizado de passagem de plantão e avaliar o efeito na quantidade de informações transmitidas na enfermaria de pediatria do HC Criança. Com o desenho prospectivo de intervenção da implementação de um sistema padronizado de passagem de plantão (I-PASS), foram realizadas gravações das passagens de plantão em período pré- e pós-intervenção. O treinamento dos médicos que concorrem às escalas de plantão foi feito em módulo teórico-prático com auxílio de plataformas digitais. A padronização da passagem de plantão resultou em aumento significativo do número de informações relevantes entregues sem prejuízo na eficiência relacionada à duração da transferência. O treinamento do protocolo aliado à discussão sobre estratégias de comunicação implicou em um maior foco e atenção durante as passagens, aumentando a proporção relativa do tempo líquido de transferência, período descontado de interrupções alheias ao momento. No que se refere ao I-PASS, houve importante acréscimo na citação de pendências e plano de contingenciamento. Assim, a padronização da passagem de plantão traz maior eficiência ao processo no que se refere a número de informações, objetividade na transferência e atenção aos pontos importantes, sendo a explanação teórica associada a práticas de simulação boas estratégias para o treinamento protocolar dos envolvidos.

The handoff, the act of transferring information and responsibility among physicians, is recognized as one of the key elements for the quality of service provided and patient safety. In 2014, a multicenter intervention study was published that resulted in a mnemonic, the I-PASS (Illness severity, Patient summary, Action list, Situation awareness and contingency planning, Synthesis by receiver), whose standardized use resulted a decrease of 23% in medical errors and of 30% in preventable adverse events in a pediatric population. This study aimed to implement this standardized handoff system and assess the effect on the amount of information transmitted at the HC Criança pediatric infirmary. With the prospective intervention design of the implementation of a standardized handoff system (I-PASS), recordings of handoffs were made in the pre- and post-intervention periods. The training of physicians who compete for shifts on duty was carried out in a theoretical-practical module with the aid of digital platforms. The handoff standardization resulted in a significant increase in the number of relevant information delivered without prejudice to the duration related to the transfer. The protocol training, together with the discussion about the communication, implicit in a greater focus and attention during the transfers, increases the relative proportion of the net transfer time, defined as period of interruptions beyond the moment. About the I-PASS, there was an important increase in the citation of action list and the contingency planning. Thus, a handoff standardization brings greater efficiency to the process about a number of information, objectivity in the transfer and attention to important points, with the theoretical explanation being associated with simulation practices of good strategies for the protocol training of those involved.
