O presente estudo insere-se no campo da saúde coletiva, na temática de organização tecnológica do trabalho em saúde. O objetivo do mesmo é analisar e interpretar como se dá o processo de produção de cuidados na Atenção Primária em Saúde (APS) aos indivíduos adultos portadores de Diabetes Mellitus tipo 2 (DM2). O DM é um importante problema de saúde pública com dimensões epidêmicas tanto no mundo quanto no Brasil. A Organização Mundial da Saúde e o Ministério da Saúde vêm incentivando a adoção de políticas e estratégias para o desenvolvimento das ações de cuidado dos portadores de DM no contexto da APS. O estudo se sustenta no aporte teórico da organização de serviços e sistemas de saúde na perspectiva da Atenção Primária à Saúde, da DM como Condição Crônica e do processo de trabalho em saúde. O cenário de pesquisa foi um Núcleo de Saúde da Família (NSF) do município de Ribeirão PretoSP, sendo realizada a coleta de dados entre janeiro de 2007 e abril de 2008. Utiliza-se de uma abordagem metodológica qualitativa, com diferentes técnicas de captação do empírico: observação participante, entrevista semi-estruturada, prontuários individuais, sistemas de informação oficiais do Ministério da Saúde, buscando-se aprofundar na análise/interpretação do processo de cuidado, que envolve um conjunto de tecnologias (duras, leve-duras e leves). Na análise do material foram identificados três temas: a política municipal de saúde para a atenção básica; o Programa de Doenças Crônico Degenerativas Diabetes e o processo de cuidado ao usuário diabético no NSF. Dentro dos principais resultados temos que a política municipal de saúde para a atenção básica proposta para o período 2010-2013, prevê a expansão em 50%, da cobertura populacional pela Estratégia de Saúde da Família (ESF), 100% pela Estratégia de Agentes Comunitários de Saúde (EACS) e 50% pela Estratégia de Saúde Bucal (ESB), mas tem encontrado limitações no que diz respeito à contratação de recursos humanos, além da insuficiência na estrutura física das unidades. Em relação ao Programa de Doenças Crônico Degenerativas Diabetes, os técnicos do nível central de gestão consideraram como a principal atividade do programa o cadastramento no SisHiPERDIA, onde persistem dificuldades com 22,2% do potencial de usuários portadores de DM cadastrados no município. Já a análise do processo de cuidado ao usuário diabético no Núcleo de Saúde da Família (NSF) revelou a importância das reuniões de discussão de famílias e administrativas para o processo de trabalho do NSF, as deficiências metodológicas na realização de atividades de educação em saúde, assim como nos cuidados domiciliares e no processo de trabalho ainda centrado na atuação do médico, na realização de procedimentos e na queixa mais do que nas necessidades dos usuários. No entanto, ficaram evidenciadas as potencialidades presentes, uma vez que a equipe aponta para as possibilidades de se refletir e colocar em ação outra lógica de atenção voltada à produção de cuidados, com o estabelecimento de espaços coletivos de decisão e estabelecimento de vínculos, atendendo aos princípios e valores da Atenção Primária Renovada.

This study, in the field of collective health, looks at the technological organization in health care. The objective is to analyze and interpret the development of the Primary Health Care (PHC) giving process for adult carriers of Type 2 Diabetes (DM2). DM is an important public health problem with epidemic dimensions in Brazil and the world. The World Health Organization and the Ministry of Health have encouraged the adoption of strategies and policies to develop health care standards for the carriers of DM within a PHC framework. This study is based on the theoretical contributions of organizational services and health systems from a PHC perspective on DM as a Chronic Condition, and the role of work in health. The research was conducted at the Family Health Center in the municipality of Ribeirao Preto SP. Data was collected from January, 2007 until April, 2008. A qualitative methodology was used with different techniques for obtaining empirical evidence: participant observation; semi-structured interviews; individual clinical histories; official information systems from the Ministry of Health, seeking to deepen the analysis/interpretation of a health care process that involves a group of technologies (hard, soft/hard and soft). In the analysis of the empirical evidence three themes were identified: The municipal health policies for Basic Medical Attention, The Program of Chronic Degenerative DiseasesDiabetes, and the health care provided to the diabetic in the Nucleus of Family Health (NFH). Among the main results we find that Basic Attention proposed for the period between 2010-2013 a 50% increase in coverage for the population by the Family Health Strategies (FHS), 100% by the Community Health Agents Strategies (EACS) and 50% by the Dental Health Strategy (ESB); but it has found limitations in relation to contracting human resources and a lack of secure structure in the units. In relation to the program of Chronic and Degenerative DiseasesDiabetes, the mid level technicians considered as a main activity the registration program in SisHIPERDIA, where difficulties persist with a 22.2% of registered carriers of DM in the municipality. The analysis of the health care process with regard to the diabetic in the NFH revealed the importance of the family and administrative group meetings for the working relationship in the unit, the poor methodologies in carrying out of activities for health education, as well as care provided at home and a work process that is still focused on the actions of the physician - fulfilling procedures and health complaints - more than on the needs of the patient. Nevertheless, potential points to follow through were made evident, once the team focuses on the possibility of reflecting and placing in action another logic that is centered on the care process, establishing with it collective decision making spaces, and the creation of ties amongst family and medical staff, keeping in mind the principles and values of a renewed Primary Care.
