O objetivo deste estudo foi avaliar a reabsorção radicular dos incisivos, após o tratamento ortodôntico fixo, para correção da mordida aberta e a correlação do grau de reabsorção radicular com os movimentos ântero-posteriores e verticais dos ápices dos incisivos, a duração de uso dos elásticos, o tempo de tratamento e as alterações dos trespasses vertical e horizontal durante o tratamento. Selecionou-se, para isso, 120 documentações, de pacientes tratados na Disciplina de Ortodontia, da Faculdade de Odontologia de Bauru - USP, pela técnica Edgewise simplificada. Essa amostra foi dividida em quatro grupos: dois com mordida aberta, compostos por 32 pacientes tratados com extração (G1), com média de idade de 14,01± 2,58 anos e 28 pacientes sem extração (G2), com média de idade 13,27± 2,75 anos, e dois grupos, com trespasse vertical normal, compostos por 30 pacientes tratados com extração (G3), com média de idade de 13,28± 1,79 anos e 30 pacientes sem extração (G4), com média de idade de 12,87± 1,43 anos. O grau de reabsorção radicular dos incisivos foi avaliado por meio de radiografias periapicais, aplicando-se escores (0-4) de acordo com a classificação de Malmgren. A movimentação dos incisivos nos sentidos ântero-posterior e vertical foi avaliada por meio das telerradiografias iniciais e finais. Para comparar o grau de reabsorção entre os grupos, aplicaram-se os testes de Kruskal-Wallis e de Dunn. Utilizou-se a correlação de Spearman para verificar a correlação do grau de reabsorção com a movimentação dos incisivos, o tempo de uso dos elásticos verticais e o tempo de tratamento. Os resultados mostraram que houve diferença estatisticamente significante do grau de reabsorção entre os grupos com mordida aberta (G1-1,57) e (G2-1,17) e entre os grupos com trespasse vertical normal (G3-1,55) e (G4-1,08). Observou-se um grau de reabsorção significantemente maior nos grupos tratados com extração (G1 e G3) que nos grupos tratados sem extração (G2 e G4), indicando que o uso dos elásticos verticais para correção da mordida aberta não influenciou no grau de reabsorção, mas sim o movimento dentário ocorrido durante a retração. Quanto ao teste de correlação entre o grau de reabsorção e a duração do uso dos elásticos e o tempo de tratamento, não houve correlação estatisticamente significante. Porém, quanto às alterações dos ápices dos incisivos centrais superiores, no sentido ântero-posterior e do trespasse horizontal durante o tratamento, a correlação foi significante (P<0,05), mas não se apresentou forte o suficiente (r= 0,29 e r=-0,27). De acordo com a metodologia empregada neste estudo, pode-se concluir que o grau de reabsorção encontrado nos grupos com mordida aberta, após o tratamento, não teve como causa a força dos elásticos verticais, mas provavelmente a movimentação dos incisivos durante o processo de retração, nos casos em que realizaram-se extrações.
Resumen en inglés
This study evaluated the root resorption of incisors after fixed orthodontic treatment for correction of open bite, as well as the anteroposterior and vertical movements of apices of incisors, duration of utilization of elastics and treatment time, for correlation with the degree of resorption. The study was conducted on 120 records of patients treated in the Discipline of Orthodontics of Bauru Dental School - USP by the simplified Edgewise technique. This sample was divided into four groups: two with open bite, composed of 32 patients treated with extraction (G1) with mean age of 14.01± 2.58 years and 28 patients without extraction (G2), with mean age of 13.27± 2.75 years, and two groups with normal overbite, composed of 30 patients treated with extraction (G3) with mean age of 13.28± 1.79 years and 30 patients without extraction (G4) with mean age of 12.87± 1.43 years. The degree of root resorption of the incisors was assessed on periapical radiographs, with application of scores (0-4) according to the classification of Malmgren. The movement of incisors in anteroposterior and vertical directions was evaluated on the initial and final cephalograms. Comparison of the degree of resorption between groups was performed by the Kruskal-Wallis and Dunn's tests. The Spearman coefficient of correlation was used to verify the correlation between degree of resorption and movement of incisors, time of utilization of vertical elastics and treatment time. The results revealed a statistically significant difference in the degree of resorption between groups with open bite (G1-1.57) and (G2- 1.17) and normal overbite (G3-1.55) and (G4-1.08). The degree of resorption was significantly higher in groups treated with extraction (G1 and G3) compared to groups treated without extraction (G2 and G4), indicating that utilization of vertical elastics for correction of open bite did not influence the degree of resorption, but rather the tooth movement occurring during retraction. Concerning the correlation between the degree of resorption and duration of utilization of elastics and treatment time, there was no statistically significant correlation. With regard to alterations in the apices of maxillary central incisors in anteroposterior direction and overjet during treatment, the correlation was significant (P<0.05) but not strong enough (r= 0.29 and r=- 0.27). According to the methodology employed in this study, it could be concluded that the degree of resorption observed in groups with open bite after treatment was not caused by the force of vertical elastics, but probably due to the movement of incisors during the retraction process.
