Constatamos no Brasil, um país de dimensões continentais, uma grande assimetria na distribuição de recursos médicos, físicos e financeiros, resultando numa heterogeneidade na qualidade, complexidade e oferta de serviços na área da saúde entre as várias regiões do Brasil. A hipótese desta dissertação é estabelecer que este problema pode ser resolvido através do uso da Telesaúde, possibilitando que centros de excelência médica ofereçam serviços de saúde à distância à instituições aonde existam carência nestes serviços. Ampliamos o conceito da Telesaúde como a oferta de serviços de saúde à distância, de forma eletrônica, quando a carência entre uma das partes, utilizando-se tecnologias livres, (protocolos, ferramentas e plataformas). A validação deste conceito ampliado de Telesaúde será aplicada à área de oncologia infantil, através da implementação e validação de um Sistema de Informação em Saúde (SIS) para a gestão da atenção em oncologia pediátrica. Oncologia Pediátrica é um assunto relevante no Brasil, pois trata-se da segunda causa de mortalidade infantil em várias regiões mais desenvolvidas do país. Na implementação de Sistema de Informação em Saúde são utilizados novos paradigmas e tendências tecnológicas, como os padrões de informação, orientação a objetos, componentes de software e arquiteturas de sistemas distribuídos, como exemplo: Java, UML, Sistemas baseado em WEB, IP e Aglomerados (Cluster LINUX).

We evidence in Brazil, a country of continental dimensions, a great symmetry in the distribution of medical, physical and financiers resources, resulting in a heterogeneity in the quality, complexity and offer of services in the area of the health among some regions of Brazil. The hypothesis of this work is to establish that this problem can be solved through the use of the Telehealth, making possible that centers of medical excellency health offer services of long-distance to the institutions where exist lack in these services. We extend the concept of the Telehealth as the offer of health services of long-distance, of electronic form, when the lack among one of the parts, using free technologies (protocols, tools and platforms). The validation of this concept extended of Telehealth, will be applied to the area of pediatric oncology, through the implementation and validation of a Healthcare Information Systems (HCIS) for the management of the attention in pediatric oncology. The pediatric oncology is a relevant subject in Brazil, therefore it is about the second cause of infantile mortality in some developed regions more of the country. In the implementation of Healthcare Information Systems, new paradigms and technological trends are used, as the standards of information, orientation the objects, components of software and architectures of distributed systems, as example: Java, UML, Systems based in WEB, IP and "Cluster LINUX".
