A violência doméstica de gênero é um problema de saúde pública que se mantém invisível nos serviços de saúde, inclusive na Atenção Primária à Saúde (APS). Os serviços de APS se inserem no território, são a porta de entrada para o Sistema Único de Saúde (SUS), e são buscados intensamente pelas mulheres em situação de violência doméstica. Alguns profissionais associam os problemas de saúde dessas mulheres a situações de violência, mas o tema é raramente trabalhado como objeto do setor da saúde, e persiste a recusa tecnológica à violência doméstica como pertinente também à saúde. O objetivo deste estudo é analisar se as representações sociais de gênero e direitos humanos das mulheres atendidas e dos profissionais de saúde da APS modificam as práticas de cuidado às mulheres em situação de violência doméstica de gênero. Para tanto, utilizou-se a metodologia qualitativa, com a Teoria das Representações Sociais por referencial metodológico para a análise do material empírico, composto de entrevistas semiestruturadas realizadas com quatro usuárias em situação de violência doméstica de gênero (mais os prontuários da família das pacientes) e 13 profissionais de saúde responsáveis pela assistência dessas mulheres na APS. A pesquisa foi realizada em quatro Unidades Básicas de Saúde (UBS) da região Oeste do Município de São Paulo: duas com a assistência em violência organizada e duas sem a assistência em violência organizada. Os casos analisados foram indicados pelas UBS. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa da FMUSP e pela Secretaria Municipal de Saúde de São Paulo. Foram resguardados os princípios éticos da pesquisa. Os resultados mostram que as mulheres e as profissionais identificam as desigualdades de gênero, ainda que as mulheres ressaltem aspectos do espaço privado, e as profissionais, do espaço público. A saúde é compreendida pelas profissionais como um direito, que só é plenamente alcançado quando outros direitos fundamentais são acessados. Em relação à assistência, os casos são percebidos no território, porém, são trabalhados apenas quando as mulheres pedem ajuda diretamente, ou se há crianças expostas, principalmente se estas também estão em situação de violência. A assistência mostrou-se imediatista, com ofertas que não escutam e pouco garantem a autonomia das mulheres. Na presença das crianças, as profissionais de saúde se sentem responsáveis pelo caso e o foco assistencial tende a ser a busca do restabelecimento da família, invisibilizando a mulher e suas necessidades, com pouca incorporação de defesa da equidade de gênero e dos direitos humanos na assistência. As profissionais de saúde da APS têm pouca articulação com a rede de serviços para o enfrentamento da violência contra as mulheres. A delegacia ainda é o principal serviço ser ofertado. Por outro lado, foram identificadas potencialidades, tais como a mudança da representação social das mulheres sobre saúde quando a assistência ofertada se pauta na integralidade e na compreensão ampliada de saúde. A escuta ativa e a compreensão dos condicionantes envolvidos na situação de violência têm potencial para barrar possíveis situações de violência institucional que se colocam, principalmente, quando há crianças envolvidas na situação de violência. Apesar dos obstáculos observados, a perspectiva de gênero e direitos humanos tem potencial para aumentar a permeabilidade da violência para dentro das práticas assistenciais às mulheres, ampliando a escuta ativa e contribuindo para a promoção dos direitos humanos no interior da atenção primária

Domestic gender violence is a public health issue that remains invisible in health services, including Primary Health Care (PHC). PHC services are a community based facilities and the gateway to Unified Heatlh System (SUS), intensely sought by women experiencing domestic violence. Some professionals associate domestic violence with women's health problems, however, this is rarely addressed as an issue of the health sector and technological refusal on domestic violence persists on health care. This study aims to analyze whether gender and human rights social representations of health professionals and women assisted on PHC modify care practices of women experiencing domestic gender violence. Qualitative methodology was used, with Social Representations Theory as a conceptual framework to analyze the empirical data - composed by semi-structured interviews with four female users under domestic gender violence situation (and medical records of patient's family) and with 13 health professionals responsible for these women's assistance in PHC. This research was conducted in four Basic Health Units (BHU) in the western region of São Paulo city: two with violence assistance service and two that did not present a structured violence assistance model. The cases analyzed were indicated by the BHU. The research was approved by the Research Ethics Committee of FMUSP and the Municipal Health Department of São Paulo. The ethical principles of the research were followed. The results reveal that women and professionals identify gender inequalities, even though women emphasize aspects of the private space and professionals emphasize public space. Health is understood by professionals as a right, which is only fully achieved when other fundamental rights are accessed. Regarding assistance, cases are identified in the territory, although they are addressed only when women directly ask for help or if there are children exposed, especially if they are also in a violence situation. The assistance was just immediate, and do not respect or guarantee women's autonomy. When it involves children, health professionals feel responsible for the case and the assistance is focused on family reestablishment, making women and their needs invisible, with a limited incorporation of gender equity and human rights defense in the care provided. PHC health professionals have little articulation with the service network to address violence against women. The police station is still the main service to be offered. On the other hand, potentialities were identified, such as changes in women's social representation about health, as offered assistance is based on comprehensive health care and the expanded understanding of the health concept. A close listening and the understanding of factors involved in violence situations are powerful tools to prevent institutional violence episodes, especially when children are involved in the cases. Despite the observed obstacles, gender and human rights perspective increase violence visibility on the assistance practices for women, increasing close listening and contributing to promote rights in the primary health care settings
