Esta Tese toma como objeto o Hospital Miguel Couto em Natal (RN), entre 1927 e 1955. Nosso objetivo central é "desconstruir" certa narrativa hegemônica sobre o hospital que buscava replicar, no âmbito local, a perspectiva progressista-humanista da medicina do séc. XX, segundo a qual o hospital era o lugar da cura dos corpos enfermos ou danificados, marcado pela inserção da tecnologia médica e pela gestão organizativa moderna da ciência. Entendemos que tal narrativa, em verdade, sustentava uma topologia histórica em que o Hospital Miguel Couto ganhou centralidade na assistência à saúde local ao longo da primeira metade do século XX, subordinando, inclusive, os demais espaços hospitalares, pelo menos até a criação da Divisão de Organização Hospitalar, em 1941-2, quando a proposta de estruturação em rede da assistência hospitalar enfraqueceu, em certa medida, seu predomínio. Conjugamos na análise das fontes ferramentas conceituais da desconstrução derridiana, como "fantasma" e "hostipitalidade", e do neopragmatismo de Richard Rorty, qual "redescrição" e "rede de alianças", buscando desmontar a narrativa do progresso médico-hospitalar e concomitantemente liberando outras representações até então recalcadas. Do ponto de vista da inserção acadêmica e disciplinar, nosso trabalho se articula na interface com a Saúde Coletiva, encarada aqui como um campo de conhecimentos, um "núcleo de saberes e práticas", fundamentando-nos na percepção de que as instituições, os discursos e as práticas da Medicina são também práticas históricas, ligadas a determinadas demandas sociais, a um mundo de relações complexas que não se fecham na corporação dos médicos com suas exigências de êxito técnico. A relação saúde-doença-sociedade possibilita o estudo da espacialidade hospitalar do Miguel Couto, na medida que o nosocômio se constituiu como parte de uma determinada organização dos serviços em saúde, encarada em sua historicidade, ao longo de quase três décadas. Para desenvolver esse estudo, segmentamos o trabalho em cinco capítulos. O primeiro deles voltou-se para as questões teóricometodológicas em particular, explicitando nossas escolhas em termos de campo, autores, conceitos e caminhos de pesquisa adotados. O segundo buscou articular a narrativa hegemônica sobre o Hospital Miguel Couto presente na documentação oficial, esboçando, já de saída, as inconsistências de uma tal representação do hospital. No capítulo 3, apostamos metodologicamente na reconstrução de quatro narrativas paralelas a respeito de aspectos da história do Miguel Couto, visando desmontar a imagem dominante de um espaço hospitalar "neutro" e voltado para a cura dos pacientes e revelar outras possibilidades interpretativas, como o caráter ativo e "político" que acompanhou a instituição em sua trajetória. Nos dois últimos, abordamos o tema da hospitalidade no interior do Miguel Couto, reconstruindo, quando possível, certo "repertório sensível" nas relações entre corpo médico e pacientes, observando as variações históricas dessa preocupação com o cuidado e o bem-estar dos doentes. Por último, e à guisa de conclusão, pudemos perceber o caráter histórico e contextual do Hospital Miguel Couto como organização de saúde, que atravessou a primeira metade do séc. XX e suas profundas transformações sociopolíticas, econômicas e culturais, erigindo-se na assistência médico-hospitalar local como instituição centralizadora e dominante nas ações de saúde pública

This Thesis takes as its object the Miguel Couto Hospital in Natal (RN), between 1927 and 1955. Our main objective is to "deconstruct" a certain hegemonic narrative about the hospital that sought to replicate, at the local level, the progressive-humanist perspective of 20th century medicine, according to which the hospital was the place of cure for sick or damaged bodies, marked by the insertion of medical technology and the modern organizational management of science. We understand that this narrative, in fact, supported a historical topology in which the Miguel Couto Hospital gained centrality in local health care throughout the first half of the 20th century, even subordinating the other hospital spaces, at least until the creation of the Division. of Hospital Organization, in 1941-2, when the proposal of structuring hospital care in a network weakened, to a certain extent, its predominance. In the analysis of sources, we combine conceptual tools of Derridian deconstruction, such as "ghost" and "hostipitality", and Richard Rorty's neopragmatism, which "redescription" and "network of alliances", seeking to dismantle the narrative of medical-hospital progress and concomitantly releasing other representations hitherto repressed. From the point of view of academic and disciplinary insertion, our work is articulated in the interface with Public Health, seen here as a "field of knowledge", a "core of knowledge and practices", based on the perception that institutions, the discourses, and practices of Medicine are also historical practices, linked to certain social demands, to a world of complex relationships that do not close in the physicians' corporation with its demands for technical success. The health-disease-society relationship enables the study of Miguel Couto's hospital spatiality, as the hospital was constituted as part of a specific organization of health services, seen in its historicity, over almost three decades. To develop this study, we segmented the work into five chapters. The first one turned to theoretical-methodological issues, explaining our choices in terms of field, authors, concepts and research paths adopted. The second sought to articulate the hegemonic narrative about the Miguel Couto Hospital present in the official documentation, outlining, from the outset, the inconsistencies of such a representation of the hospital. In chapter 3, we methodologically bet on the reconstruction of four parallel narratives about aspects of Miguel Couto's history, aiming to dismantle the dominant image of a "neutral" hospital space focused on patient healing and reveal other interpretative possibilities, such as character active and "political" that accompanied the institution in its trajectory. In the last two, we address the theme of hospitality within Miguel Couto, reconstructing, whenever possible, a certain "sensitive repertoire" in the relationships between the medical staff and patients, observing the historical variations of this concern with the care and well-being of patients. Finally, and by way of conclusion, we could see the historical and contextual character of Hospital Miguel Couto as a health organization, which spanned the first half of the 20th century.
