Introduction: Cranioencephalic trauma (TBI) is one of the major public health problems in the world. Diffuse axonal injury (LAD) is the most important and frequent cause of neurological sequelae in individuals with TBI. LAD occurs in multiple locations in the brain and causes symptoms that may be temporary or permanent in patients who are followed up in the long term. The objective of the present study was to analyze the social support network and the family functionality in patients with LAD and ADL. METHOD: We performed a cross-sectional and comparative study on family functionality and social support in patients six months after TBI in an outpatient trauma service. The sample consisted of 120 individuals divided into two groups, 50 patients suffering from TBI with moderate and severe diffuse axonal injury and 70 healthy volunteers constituting the control group. We used a sociodemographic questionnaire, the minimum relation map - MMR and the APGAR family test to compare the two groups. Results: In relation to the family function, we verified that there was a higher prevalence of moderate functionality in the study group (77.8%), compared to the control group. Regarding the analysis of the family provider, there was a higher prevalence of the parents as heads of families (57.5%) in the study group and higher prevalence of spouses, with the providers in the control group. We observed a significant difference in leisure activities, and the individuals in the study group presented greater impairment in these activities. The indicators of social support were evaluated by the minimum map of social relations that indicate a greater participation in the support of the community in relation to the family in the control group. Conclusion: Patients who suffered diffuse axonal injury present moderate functionality, with greater community support compared to the family. In patients with diffuse axonal injury, the patient's parents are the providers of the family
