Introdução: Pacientes com arterite de Takayasu (AT) apresentam risco elevado de síndrome metabólica e doença cardiovascular (DCV). Atualmente, não há biomarcadores bem validados para avaliar este risco nesta população. Estudos prévios em diferentes coortes relacionaram as concentrações séricas de osteoprotegerina (OPG) e seus polimorfismos à aterosclerose acelerada e a pior prognóstico cardiovascular. Dessa forma, seria de suma relevância avaliar esta proteína e seus polimorfismos como um potencial biomarcador de DCV em pacientes com AT. Objetivos: Avaliar as concentrações séricas de OPG e seus polimorfismos de nucleotídeo único (SNPs) em pacientes com AT, e associar esses parâmetros com dados clínicos. Métodos: Este estudo transversal bicêntrico incluiu pacientes com AT (ACR/EULAR 2022) que foram comparados com controles saudáveis pareados por sexo e idade. As concentrações séricas de OPG e a frequência de SNPs de OPG [1181G>C (rs2073618), 245A>C (rs3134069), 163T>C (rs3102735) e 209C>T (rs3134070)] foram comparados entre os dois grupos e associados a dados clínicos. Resultados: No total, 101 pacientes com AT e 93 controles foram incluídos no estudo. As concentrações séricas de OPG (3,8±1,9 vs. 4,3±1,8 pmol/L, respectivamente; P=0,059) e seus quatro polimorfismos foram comparáveis entre os dois grupos. Em uma análise adicional apenas de pacientes com AT, as concentrações séricas de OPG e seus quatro SNPs não foram associados a nenhum parâmetro de DCV, exceto concentrações séricas mais elevadas de OPG entre pacientes sem dislipidemia. Conclusões: Não foram observadas diferenças significativas nas concentrações séricas de OPG ou nas frequências genotípicas dos SNPs de OPG entre os grupos de pacientes e controle. Da mesma forma, não foi encontrada associação entre parâmetros laboratoriais e dados clínicos sobre risco de DCV em pacientes com AT

Introduction: Takayasus arteritis (TAK) patients are at an elevated risk of metabolic syndrome and cardiovascular disease (CVD). Currently, there are no well-validated biomarkers to assess this risk in this population. Previous research in different cohorts has linked serum levels of osteoprotegerin (OPG) and its polymorphisms to accelerated atherosclerosis and a marker of poor prognosis in CVD. Thus, it may be relevant to assess this protein as a potential biomarker of CVD in patients with TAK. Objectives: To evaluate the serum levels of OPG and its single nucleotide polymorphisms (SNPs) in patients with TAK, and to associate these parameters with clinical data. Methods: This bicentric cross-sectional study included patients with TAK (ACR/EULAR 2022) and compared them with healthy controls matched for sex and age. The serum levels of OPG and the frequency of OPG SNPs [1181G>C (rs2073618), 245A>C (rs3134069), 163T>C (rs3102735), and 209C>T (rs3134070)] were compared between the both groups and associated with clinical data. Results: A total of 101 patients with TAK and 93 controls were included in the study. The serum levels of OPG (3.8±1.9 vs. 4.3±1.8 pmol/L, respectively; P=0.059), and its four polymorphisms were comparable between both groups. In an additional analysis of only patients with TAK, serum OPG levels and its four genes were not associated with any CVD parameters, except for higher serum OPG levels in patients without dyslipidemia. Conclusions: No significant differences were observed in serum OPG levels or the genotype frequencies of OPG SNPs between the patient and control groups. Similarly, no correlation was found between laboratory parameters and clinical data on CVD risk in patients with TAK
