INTRODUÇÃO: A Síndrome da deleção 22q11.2 (SD22q11.2) apresenta alta variabilidade fenotípica incluindo diferentes características clínicas e comportamentais, representado um desafio diagnóstico ao nascimento, que pode retardar o tratamento adequado e o aconselhamento genético. O diagnóstico precoce da SD22q11.2 é fundamental, devido à significativa morbimortalidade no primeiro ano de vida, no impacto na conduta do tratamento para essas crianças e para a elaboração de políticas públicas de saúde. OBJETIVO: Investigar a frequência da SD22q11.2 em recém-nascidos (RN) e lactentes no primeiro ano de vida portadores de cardiopatia congênita (CC), atendidos no Centro Neonatal do Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (ICr-HCFMUSP); no Centro de Tratamento Intensivo Neonatal do Instituto da Criança (CTIN2) - ICr-HCFMUSP e na UTI Pediátrica do Instituto do Coração (InCor) HCFMUSP por meio da técnica de MLPA (Multiplex Ligation-dependent Probe Amplification), que detecta a variação do número de cópias gênicas. MÉTODOS: Participaram do estudo 118 RN e lactentes portadores de CC no período de março de 2012 a junho de 2014. As amostras de DNA foram analisadas utilizando a técnica de MLPA com os kits P036, P064, P070, P250-B1 e P356-A1. RESULTADOS: Nos 118 pacientes estudados (69M: 49F), a idade variou entre um dia a 11 meses, com média de idade de 41,4 dias. A análise molecular pela técnica de MLPA possibilitou a detecção da deleção 22q11.2 em 10/118 pacientes (8,5%). Outras alterações genômicas foram diagnosticadas em 6/118 pacientes (5%): deleção 1p36, deleção 7p, deleção 8p23 (2 casos), duplicação 7q e duplicação 8q24. As CC encontradas nos 10 pacientes com a SD22q11.2 foram: atresia pulmonar, estenose pulmonar, tetralogia de Fallot, truncus arteriosus, coarctação de aorta, dupla via de saída de ventrículo direito e conexão anômala de veias pulmonares. CONCLUSÕES: Foi possível investigar e detectar alterações genômicas presentes em RN e lactentes portadores de CC com a técnica de MLPA em um número maior do que é demonstrado na literatura. Isso demonstra a importância da investigação precoce em pacientes com CC grave para avaliar o prognóstico, os riscos de recorrência e, ao mesmo tempo, permitir seguimento e terapêuticas adequados, visando melhora na qualidade de vida dessas crianças

INTRODUCTION: The 22q11.2 deletion syndrome (SD22q11.2) has a high phenotypic variation, including different clinical and behavioral features, which represents a diagnostic challenge at birth, which may delay the proper treatment and genetic counseling. The early diagnosis of SD22q11.2 is crucial, due to the significant morbi-mortality during the first year of life, to the impact in conducting the treatment for these infants and to the formulation of health public policies. OBJECTIVE: Investigate the incidence of SD22q11.2 in newborns (NB) and infants with congenital heart diseases (CHD), assisted by the Newborn Care Center of the Children's Institute- HCFMUSP; by the Newborn Intensive Care Center of the Children's Institute (CTIN2) - HCFMUSP and by the pediatric ICU of the Heart Institute- HCFMUSP using the MLPA technique, which detects aberrant copy numbers. METHODS: The survey included 118 NB and infants with CHDs from March 2012 to June 2014.. The DNA samples were analyzed using the MLPA technique with the P036, P064, P070, P250-B1 and P356-A1 sets. RESULTS: The molecular analysis using the MLPA technique allowed the detection of the 22q11.2 deletion in 10/118 patients (8.5%). The other genomic alterations diagnosed in 6/118 patients (5%) were: 1p36 del, 7p del, 8p23 del (2 cases), 7q dup and 8q24 dup. In the 118 surveyed patients (69M: 49F), the age ranged from one day to 11 months old, with an average age of 41.4 days. The heart diseases found in the 10 patients with SD22q11.2 were: pulmonary atresia, pulmonary stenosis, Fallot tetralogy, truncus arteriosus, aortic coarctation, double outlet right ventricle and anomalous connection of pulmonary veins. CONCLUSIONS: It was possible to investigate and detect genomic alterations present in NB and infants with CHDs using the MLPA technique in a larger number than is demonstrated in the literature. The detection of genomic alterations will allow clinicians to evaluate the prognosis and the risks of recurrence while allowing proper follow-up and treatment, aiming at improving the children's life quality
