O reconhecimento da associação entre anticorpos anti-HLA específicos contra o doador (DSA, de donor specific antibody) e o risco aumentado de rejeição e perda do enxerto é de longa data. Pacientes previamente sensibilizados apresentam maior risco de rejeição mediada por anticorpos no pós-transplante. Pouco se sabe sobre a correlação entre os achados histológicos de biópsia precoce do enxerto renal e desfechos relacionados à função renal e à sobrevida desse enxerto. Dessa forma, não apenas a monitorização imunológica da presença de DSA, mas também o seguimento histológico, através de biópsia protocolar, podem contribuir para o diagnóstico precoce desse tipo de rejeição e avaliação prognóstica do transplante. Esse é um estudo retrospectivo, observacional de análise de uma coorte de 324 pacientes sensibilizados com transplante renal isolado de doador vivo ou falecido submetidos à biópsia protocolar nos primeiros 7 a 10 dias de transplante. O objetivo primário do estudo é correlacionar achados histológicos da biópsia protocolar e a presença de DSA pré-transplante com a evolução da função renal e a sobrevida do enxerto em até 5 anos de seguimento clínico

The association between the presence of donor specific antibodies (DSA) and the increased risk of antibody-mediated rejection and graft loss is well known in the field of kidney transplant. Although previously sensitized patients have a higher risk of antibody-mediated rejection, little is known about the correlation between histological findings in early performed renal graft biopsies and transplant outcomes related to renal function and graft survival. Therefore, not only is the immunological surveillance pre- and post-transplant important, but the histological follow-up through protocol biopsy may also contribute to the early diagnosis of rejection and to the prognosis evaluation of the kidney transplant. This is a retrospective, observational, cohort study, which included 324 sensitized isolated kidney transplant patients, with deceased or live donor, who underwent a protocol biopsy between 7 to 10 days after the transplant surgery. The primary objective of this study is to correlate the histological findings of the early protocol biopsy and the presence of DSA pre-transplant with the evolution of the renal graft function and graft survival up to 5 years of follow-up
