INTRODUÇÃO E OBJETIVO: A disfunção erétil (DE) vasculogênica pode ser avaliada pela ultrassonografia Doppler do pênis (UDP) com injeção intracavernosa (IIC). O relaxamento máximo da musculatura lisa dos corpos cavernosos é essencial para se obter um diagnóstico fidedigno. Contudo, inibições psicológicas, que cursam com liberação adrenérgica, podem dificultar esse processo. O objetivo deste estudo é avaliar o uso do estímulo sexual manual e audiovisual (EMAV) como ferramenta durante o UDP para reduzir o número de resultados falso positivos. MÉTODOS: Um total de 40 homens (média de idade 61,8 ± 10,2 anos) com DE foram envolvidos. O Índice Internacional de Função Erétil-5 (IIEF-5) foi aplicado antes do início da avaliação pelo UDP. Cada paciente realizou duas sessões de UDP com IIC de forma randômica com intervalo de 7 dias. A indução farmacológica foi feita com Alprostadil 20 mcg. A sessão A foi feita com IIC e a Sessão B com IIC e EMAV. A velocidade de pico sistólico (VPS), velocidade diastólica final (VDF) e o índice de resistividade (IR) foram aferidos nos momentos 5, 10, 15 e 20 minutos depois da IIC. O diâmetro das artérias cavernosas foi medido antes e após a IIC. RESULTADOS: 23 pacientes realizaram a primeira sessão sem EMAV e 17 com EMAV. Não houve diferenças quanto às características basais. O diâmetro das artérias cavernosas foi maior na sessão com EMAV, independentemente do número da sessão ou lateralidade. A VDF foi menor com EMAV (p=0,022); o IR foi maior (p=0,019) e a VPS não foi influenciada significativamente (p = 0,77). 4/40 pacientes tiveram o diagnóstico modificado devido ao EMAV (10,0%, 95% IC 2,8 - 23,7%). Entre aqueles com DE veno-oclusiva 3/12 (25,0%, 95% IC 5,5 - 57,2%) e entre aqueles com DE arterial 1/4 (25,0%, 95% IC 0,6 - 80,6%). Doze pacientes tiveram diagnóstico final de DE vasculogênica. Nestes, houve maior tendência a serem mais velhos (66,3 ± 5,8 vs 59,9 ± 11,2 anos, p=0,070), maior prevalência de diabetes (75,0 vs 25,0%, p=0,005) e maior propensão a dislipidemia (58,3 vs 25,0%, p=0,071) e sedentarismo (91,7 vs 60,7%, p=0,067). O resultado do IIEF foi menor nos pacientes com DE (6,3 ± 3,3 vs 12,0 ± 5,8, p=0,003) com uma boa acurácia avaliada pela área da curva ROC (AUC-ROC 0,80, 95% IC 0,66 - 0,94). O melhor ponto de corte foi <=9 (sensibilidade 83,3%, especificidade 71,4%). CONCLUSÃO: O EMAV adicionado ao UDP com IIC melhora os parâmetros do exame, inclusive mudando o diagnóstico, reduzindo o número de exames falso positivos

INTRODUCTION AND OBJECTIVE: Vascular erectile dysfunction (ED) can be assessed by drug-induced Penile Doppler Ultrasound (PDU). To obtain an accurate diagnosis using this method, maximal relaxation of the cavernous arteries and smooth muscle is mandatory. However, adrenergic discharge by psychological inhibitions may hinder the optimal response of the erectile tissue to intracavernous injection (ICI). The goal of this study was to evaluate the use of Audiovisual Sexual Stimulation (AVSS) as a tool to assist PDU in order to avoid false-positive results. METHODS: A total of 40 men (mean age 61.8 ± 10.2 years) with ED were enrolled. The International Index of Erectile Function Questionnaire (IIEF-5) score was applied before starting the evaluation. Patients were randomized to PDU sessions with and without AVSS in different regimens. ICI was done with 20 mcg of Alprostadil. Session A was performed under ICI alone. Session B was performed under ICI and AVSS. The interval between sessions was 7 days. Penile vascular parameters including peak systolic velocity (PSV), end diastolic velocity (EDV), and resistive index (RI) were measured 5, 10, 15 and 20 minutes after ICI. Cavernous arteries diameters were measured before and after pharmacological induction. RESULTS: 23 patients performed the first session without AVSS and 17 performed the first session with AVSS. There were no differences on baseline characteristics. The cavernous arteries diameters were greater in the session with AVSS regardless of session sequence and laterality. EDV was always lower (p = 0.022) and the RI was always higher (p = 0.019) in the AVSS session. PSV was not influenced by AVSS (p=0.768). The proportion of patients whose diagnosis was changed due to the AVSS intervention was 4/40 (10.0%, 95% CI by the binomial exact method: 2.8 - 23.7%). Out of 12 patients with venous ED on the PDU without AVSS, 3 had their diagnosis changed to normal following AVSS (25.0%, 95% CI 5.5 - 57.2%). Out of 4 men with arterial ED on the PDU on ICI alone, 1 had normal hemodynamics after AVSS (25.0%, 95% CI 0.6 - 80.6%). Twelve patients had a final diagnosis of ED on the Doppler evaluation. These had a higher tendency to be older (66.3 ± 5.8 vs 59.9 ± 11.2 years, p=0.070), had a higher prevalence of diabetes (75.0 vs 25.0%, p=0.005) and a higher propensity for dyslipidemia (58.3 vs 25.0%, p=0.071) and sedentarism (91.7 vs 60.7%, p=0.067). The IIEF score was lower on the ED group (6.3 ± 3.3 vs 12.0 ± 5.8, p=0.003) and had a good accuracy as evaluated by the area under the ROC curve (AUC-ROC 0.80, 95% CI 0.66 - 0,94). The best cut-off score was <=9 (sensitivity 83.3% and specificity 71.4%). CONCLUSIONS: Adding AVSS during PDU improves ICI response and may help the physician more accurately evaluate penile hemodynamics
