Introdução: A doença do refluxo gastroesofágico (DRGE) é a enfermidade mais comum do trato gastrointestinal com estudos epidemiológicos indicando prevalência de 10 a 20% na população de pacientes de países ocidentais. Como a maioria dos pacientes com sintomas de refluxo apresentam endoscopia digestiva alta (EDA) normal, tentativas têm sido levantadas para avaliar novas técnicas endoscópicas para identificar alterações não detectadas na endoscopia padrão. A endoscopia de alta definição associada a Narrow Band Imaging (NBI) pode proporcionar uma avaliação mais detalhada da mucosa, permitindo a identificação de microerosões no esôfago distal. Objetivo: Validar a presença e o significado das microerosões da mucosa esofágica distal por endoscopia de alta definição e NBI em biópsias de pacientes com sintomas típicos de refluxo gastroesofágico e sua possível associação na fisiopatologia na DRGE. Casuística e Métodos: Após critérios de inclusão e exclusão, foram selecionados 70 pacientes do ambulatório de Gastroenterologia do Hospital das Clínicas da Universidade de São Paulo. A avaliação endoscópica foi sequencialmente realizada à pHmetria prolongada e manometria esofágica. No exame endoscópico, foram realizadas biópsias de mucosa esofágica conforme protocolo estabelecido, seguindo-se análise histopatológica por microscopia óptica e imuno-histoquímica nos pacientes analisados. Resultados: Dos 70 pacientes analisados, 30 pacientes apresentaram microerosões (42,9%) e, em 40 pacientes (57,1%), não foram observadas microerosões. A phmetria prolongada não evidenciou achados estatísticos significativos nas variáveis estudadas. No estudo de manometria esofágica, tanto em relação aos valores pressóricos como no diagnóstico, não foi verificada diferença estatística entre pacientes com microerosões e sem microerosões (controles). O escore histológico definido para a mucosa esofágica, com todos os seus componentes, não mostrou diferença estatística significante entre os pacientes com e sem microerosões endoscópicas. A avaliação quantitativa celular por meio de imuno-histoquímica da mucosa esofágica foi realizada em 80% dos pacientes (56/70), sendo 48,21% (27/56) com microerosões positivas e 51,79% (29/56) com microerosões negativas. No estudo da mucosa esofágica houve apenas uma célula imunomarcada IL1Beta em um único paciente. No estudo da IL6 e IL8 não houveram diferenças estatísticas significativas em relação ao número total de células imunomarcadas e em relação ao número de células por área (Micro m²) tecidual avaliada entre os pacientes com e sem microerosões. Conclusão: A presença de microerosões esofágicas observadas em endoscopia de alta definição com NBI em pacientes com sintomas típicos da DRGE, não evidenciou diferença estatística significativa nos estudos pHmétricos, manométricos, histológicos e de imunohistoquímicos em relação aos controles

Introduction: Gastroesophageal reflux is the most common gastrointestinal disease with a prevalence of 10 to 20% in western countries. Most patients with symptomatic gastroesophageal reflux disease (GERD) have a normal upper gastrointestinal endoscopy. Attempts have been made to define new endoscopic techniques for identification of abnormalities non detected in standard endoscopic exam. The high definition endoscopy with NBI could achieve a more detailed mucosal evaluation, allowing distal esophageal microerosions identification. Objectives: Validate the presence and the meaning of distal esophageal mucosal microerosions using high definition endoscopy and NBI through esophageal biopsy in gastroesophageal reflux disease symptomatic patients and the association with GERD physiopathology. Casuistry and Methods: 70 selected patients from the gastroenterology outpatient clinic (University of Sao Paulo Hospital). Endoscopic evaluation was sequentially performed after the pHmetry and esophageal manometry. Esophageal mucosal biopsies were obtained following established protocol for histological and immunohistochemical study. Results: From 70 patients, 30 patients showed mucosal microerosions (42,9%) and in 40 patients no microerosions were detected (57,1%). Prolonged pHmetry did not revealed significant statistical findings in the studied variables. In the esophageal manometry study, both in relation to pressure values and in the diagnosis, no statistical difference was verified between patients with microerosions and without microerosions (controls). The histologic score defined for esophageal mucosa with all its components did not revealed significantly statistical difference considering patients with or without endoscopic microerosions. The quantitative cellular evaluation by immunohistochemistry of the esophageal mucosa was performed in 80% of the patients (56/70), 48.21% (27/56) with positive microerosions and 51.79% (29/56) with negative microerosions. Regarding the quantitative result, there was only one immunolabelled cell with the anti-IL-1Beta antibody in esophageal mucosal biopsies in a single patient of the 56 patients at this stage of the study. In the IL6 and IL8 studies, there were no significant statistical difference in relation to the total number of cells immunolabelled and in relation to the number of cells per tissue area (Micro m²) evaluated between patients with and without microerosions. Conclusion: The presence of esophageal microerosions observed in high-resolution endoscopy with NBI in patients with typical symptoms of GERD did not show a statistically significant difference in pHmetric, manometric, histological and immunohistochemical studies in relation to controls
