Foram estudados 656 casos de tuberculose, referentes ao ano de 1984, envolvendo população residente no Município de São Paulo, na faixa etária de menores de 15 anos, cujo diagnóstico foi notificado ao Centro de Informaçôes da Saúde da Secretaria de Estado da SaGde (SP). São descritas algumas características gerais, referentes, à região de residência, faixa etária, sexo, formas de tuberculose e localização em diferentes órgãos e/ou sistemas, positividade ao exame bacteriológico, instituição que fêz a notificação e mês do ano em que o diagnôstico de tuberculose foi feito. Em cada caso, além de se levar em consideração algumas características próprias do médico que efetuou a notificação, o diagnóstico foi estudado tendo em vista: o local de sua realização, os elementos disponíveis e o critério utilizado. Os resultados mostram que predominou, na população de estudo, a tuberculose pulmonar, tanto a isolada quanto a associada a tuberculose extrapulmonar. A tuberculose miliar ocorreu em 3,7% dos casos, independente de ser considerada isolada ou associada a outras localizações. A meningite tuberculosa e a tuberculose pleural aparecem entre as localizações extrapulmonares mais frequentes. Em relação ao grupo etário, a maior incidência de casos ocorreu em menores de 5 anos. Em 75% da população de estudo a probabilidade de acerto no diagnóstico, avaliado por um sistema de escore, foi elevada. A notificação dos casos foi realizada por médicos de várias especialidades, predominando a que foi feita por pneumologistas, quando consideradas as especialidades principais (isoladas ou as saciadas). Na população de estudo, 30% dos casos foram notificados por médicos que julgaram ser seus conhecimentos insuficientes para trabalhar com a tuberculose na criança. Entre os casos notificados, 70% o foram por médicos que referiram dificuldades para fazer o diagnóstico de tuberculose na faixa etária de menores de 15 anos.

The present study describes 656 cases of tuberculosis notified to the Center of Health lnformation of the Secretaria de Estado da Saúde of the State of São Paulo, Brazil, during 1984; all individuais in the population aged less than 15 years and lived in the city of São Paulo. The characterization of the population included: age, sex, place of living, the form of tuberculosis, its localization (in different organs and/or systems), positiveness of bacteriological examination, as well as the features of the reporting health services and the month when the diagnosis was done. For each notification, besides considering some aspects of the physician's background, it was taken into account for the diagnosis; the criterion utilized, the available elements and the institution from where it carne from. A predominance of pulmonary disease is demonstrated; it occurs either alone or associated to an extrapulmonary tuberculosis. In spite of being isolated or not to others tuberculosis' localizations, the miliary tuberculosis is responsible for 3,7% of the cases. Among the extrapulmonary tuberculosis, the meningitis and pleural tuberculosis were found more frequent. Considering all cases studied, it is shown that tuberculosis predominantes among children in the age group of 0-4 years. A high degree of accuracy for the diagnosis was guaranteed for 75% of the studjed population, by utilizing a special scoring system. The cases studjed were notified by physicians of different specialties, however predominating the pneumologists when the major specialty is considered. For 30% of the cases, the own physician declared himself to have some lack of knowledge needed for dealing with child tuberculosis. On the other hand, 70% of the cases were notified by physicians declaring to have found difficulties to perform the right diagnosis for children aged less than 15 years.
