No Brasil os modelos de atenção à saúde, em particular os direcionados à promoção da saúde da população infantil, têm sido caracterizados por um enfoque primordialmente direcionado aos determinantes biológicos, e enfrenta diversas dificuldades na sua implementação. Vários fatores podem ser relacionados a essa problemática, tais como carência de recursos materiais e humanos, falta de infraestrutura organizacional e de gestão, além dos aspectos socioculturais imbricados na adoção ou não das práticas de promoção à saúde. Essa Tese de doutorado teve como objetivos: descrever as práticas de promoção da saúde desenvolvidas numa comunidade escolar do Município do Rio de Janeiro à luz das políticas públicas e da cultura escolar; analisar os significados atribuídos pelos escolares a essas práticas; e, discutir o modelo de atenção e as práticas de promoção da saúde desenvolvidas na escola, a partir dos significados atribuídos pelos sujeitos de estudo a essas práticas. A abordagem qualitativa a partir da perspectiva etnográfica apoiada nos pressupostos de Geertz permitiu compreender os significados emergentes das produções simbólicas (desenhos e seus respectivos relatos) acerca do tema saúde. Os resultados demonstraram que as ações de promoção da saúde estão incorporadas ao cotidiano da escola, dialogando com as políticas setoriais, em que pese à centralidade dessas ações ainda em aspectos predominantemente normativos. Em relação aos significados sobre promoção da saúde atribuídos pelos escolares destaca-se que estes reproduziram clichês médico-sanitários constantes tanto do modelo hegemônico (biomédico e higienista) como da cultura escolar. As produções simbólicas retrataram com riqueza de detalhes, criatividade e imaginação os espaços sociais de desenvolvimento das práticas de saúde, os recursos materiais utilizados e os procedimentos envolvidos, bem como os atores sociais participantes dessas ações. Por outro lado, também emergiram significados, ainda que menos frequentes, que criticaram o modelo assistencial vigente, apontando para uma visão mais ampliada de saúde a partir da perspectiva sociocultural. Os resultados dessa Tese serviram ainda para reforçar a influência da cultura local, dos hábitos e dos modos de vida na construção do conceito de saúde por parte da coletividade estudada

In Brazil, the health care models , particularly those aimed at the promotion of children´s health, have been characterized by its focus which is mainly directed to biological determinants, and face many difficulties in its implementation. Several factors may be related to this problem, such as deficiency of material and human resources, lack of organizational and management infrastructure, in addition to the socio-cultural aspects which will determine the adoption or not of the health promotion practices. . The objectives of this Thesis were to describe the practices of health promotion developed in a school in the city of Rio de Janeiro under the light of public policy and educational culture; to analyze the meanings attributed by the students to these practices, and to discuss the model care and health promotion practices developed at school, based on the meaning of the students drawings. The qualitative approach from the ethnographic perspective supported by Geertz assumptions allowed the understanding of the meanings emerging from the symbolic productions (drawings and their respective reports) on the health theme. The results showed that the health promotion actions are already part of the school routine, dialoguing with the sectorial policies, although these actions are still predominantly attached to normative aspects. Regarding the meanings of the collected material on health promotion one should notice that it represented medical-sanitary clichés present on both the hegemonic model (biomedical and hygienist) and the school culture. The symbolic productions portrayed in detail, creativity, and imagination the social spaces for the development of health practices, the material resources, and procedures involved, as well as the social actors participating in these actions. On the other hand, although less frequently, some productions emerged criticizing the present model, pointing to a broader view of health from the socio-cultural perspective. The results of this Thesis reinforce the influence of the local culture, habits and ways of life in the construction of the concept of health by the collectivity studied
