Este estudo tem por objeto a construção dos projetos terapêuticos realizados pelos profissionais de um Centro de Atenção Psicossocial - CAPS III. Considera o processo de criação e expansão do Sistema Único de Saúde-SUS e da Reforma Psiquiátrica no país, cujos princípios e práticas definem a atenção descentralizada, mediante ações próximas das necessidades e realidades das pessoas com sofrimento psíquico, a participação da família e da comunidade. Nesse contexto, os trabalhadores do CAPS têm o desafio de produzir um cuidado segundo um projeto terapêutico individual, construído com a família e equipe, que considere as singularidades da vida da pessoa. Esse modo de cuidar coloca em discussão o olhar do modelo centrado na doença em benefício da ampliação da clínica deslocando, assim, o foco para o processo centrado na pessoa e sua situação real de vida. Considerando a complexidade dessa ação, e na perspectiva de contribuir no contexto das atuais transformações da atenção em saúde mental, esta pesquisa teve por objetivo analisar e descrever as potencialidades e dificuldades na construção dos projetos terapêuticos realizados pelos profissionais de um CAPS III. Com base no método cartográfico e na técnica do grupo focal, buscou-se captar as múltiplas dimensões do objeto. Participaram do estudo trabalhadores das equipes diurna e noturna de um CAPS III do município de Diadema-SP. A condução do grupo focal ocorreu a partir de roteiro pré-elaborado, conforme os pressupostos de Minayo. Foram realizados dois grupos com a equipe noturna e três com a equipe diurna, com a discussão de três casos, neste contexto entendido como a história de um sujeito em sofrimento psíquico sob os cuidados das equipes nos CAPS. A análise foi realizada a partir dos dados provenientes das discussões dos trabalhadores no grupo focal. Dois casos foram selecionados: um discutido com a equipe diurna e o outro, com noturna. Nesse processo a pesquisadora focalizou sua atenção nas informações referentes a construção de projetos terapêuticos pelos sujeitos da pesquisa. No processo de análise construiu-se o Fluxograma Analisador, a partir dos dados colhidos nos grupos, para caracterizar as intervenções realizadas e as relações estabelecidas, o que permitiu evidenciar os nós críticos no processo de trabalho das equipes. Dessa maneira, podemos constatar que os principais desafios a serem enfrentados pela equipe na construção dos projetos terapêuticos são: constituição de uma integração efetiva entre os diversos profissionais da equipe e entre os diferentes plantões; organização de espaços sistemáticos para construção e reavaliação dos projetos entre a equipe; real mudança do paradigma sobre o processo saúde-doença ainda enraizado nos pressupostos ao paradigma biomédico; registros mais rigorosos das intervenções dos profissionais nos prontuários, para facilitar a circulação das informações entre a equipe e a tarefa do técnico de referência na composição e reavaliação dos projetos; instituição de espaços de supervisão para trabalhar os sentimentos da equipe no cuidado a pacientes graves. Porém, mesmo diante desses desafios, os trabalhadores esforçavam-se em construir um trabalho permeado pelas proposições da reabilitação psicossocial e muitos questionaram pontos considerados falhos nesse processo trabalho, o que evidencia a potência desses profissionais.

This study focuses on the construction of the projects made professionals in a Psychosocial Attention Center - CAPS III. Does the process of creation and expansion of the Unified Health System - SUS and Psychiatric Reform in Brazil, whose principles and practices define the decentralized attention through actions closer to the needs and realities of people with psychological distress, family involvement and community. In this context, workers in CAPS have the challenge of producing a therapeutic care, according to an individual project, built with family and staff, to consider the uniqueness of the person's life. This mode of care calls into question the look of the model based on disease in favor of expanding the clinical shifting, so the focus to the process centered on the person and his real-life situation. Considering the complexity of this action, and in order to contribute in the context of current transformations of care in mental health, this research was to analyze and describe the strengths and difficulties in the construction of the projects made by therapeutic professionals from a CAPS III. Based on the mapping method and the focus group technique, we attempted to capture the multiple dimensions of the object. Study participants were employees of the teams day and night of a CAPS III of the Municipality of Diadema-SP. Conducting the focus group occurred from pre-prepared script, as the assumptions of Minayo. Two groups were conducted with the team overnight and three with the team during the day, with a discussion of three cases in this context understood as the history of a subject in psychological distress in the care of the teams in CAPS. The analysis was based on data from the discussions of workers in the focus group. Two cases were selected: a discussion with the team and the other daytime, with nighttime. In this process the researcher focused his attention on information concerning the development of therapeutic projects by the research subjects. During the review process built the flowchart Analyzer, based on data collected in groups, to characterize the interventions and the relationships, which highlighted the critical nodes in the process of work teams. Thus, we note that the main challenges faced by the team in the construction of therapeutic projects are: establishment of an effective integration between the various health professionals and between different shifts, systematic organization of spaces and revaluation of building projects between team; real paradigm change on the health-disease is still rooted in assumptions to the biomedical paradigm, more stringent records of assistance from professionals in the medical records to facilitate the flow of information between the team and the task of technical reference in the composition and re- projects; spaces supervisory for the feelings of the team working in the care of critically ill patients. However, even in the face of these challenges, workers struggled to build a work permeated by the propositions of psychosocial rehabilitation and many have questioned points considered flawed in that process work, showing the power of these professionals.
