Introdução Estudos têm evidenciado a importância da prática regular de atividade física na hipertensão arterial. Objetivos Avaliar o efeito de atividades com uso do pedômetro, orientadas pela enfermagem, na prática de atividade física e na mudança de comportamento em hipertensos acompanhados em Estratégia Saúde da Família. Métodos Realizou-se estudo prospectivo, experimental, durante 10 semanas, em uma unidade básica de saúde da Região Oeste da cidade de São Paulo. Foram estudados 263 hipertensos sedentários acompanhados na Estratégia Saúde da Família (70,3% mulheres, 57,4% não brancos, 56,4±9,6 anos). Os hipertensos classificados como inativos pelo Questionário Internacional de Atividade Física (IPAQ) foram divididos em Grupo 1- Controle (G1, n=130) e Grupo 2 - Intervenção (G2, n=133). Os hipertensos dos dois grupos foram avaliados no início e no final do estudo em relação a nível de atividade física, estágio de mudança de comportamento, medida casual da pressão realizada pela enfermeira e medida residencial da pressão arterial (MRPA), com aparelho automático validado, e medida dos passos durante três dias com o uso do aparelho pedômetro. Somente os hipertensos do grupo G2 receberam orientação da enfermagem para incentivo à mudança de hábito frente à atividade física e uso do pedômetro durante dez semanas consecutivas. Resultados No final do estudo, nos hipertensos dos dois grupos estudados (G1 e G2), observou-se que houve mudança estatisticamente significante (p<0,05) nas seguintes variáveis: redução da pressão arterial casual (G1-139,4±19,1/83,1±12,1 vs 130±15,2/79,1±10 e G2-138,7±19/83,6±10,9 vs 128,7±14,8/76,2±10,7 mmHg) e da medida residencial (G1-131,59±18,21/78,12±9,49 vs 126,71±14,74/78,30±10,57 e G2-131,41±17,95/79,35±9,27 vs 126,54±13,51/76,29±8,99 mmHg), exceto para a MRPA diastólica dos hipertensos do G1; aumento no número de passos, sendo maior nos hipertensos do G2 no final (6331±3202 vs 8108±3103 passos); redução no índice de massa corpórea e circunferência abdominal, porém a circunferência abdominal dos homens do grupo G2 no fim do estudo foi menor que a do G1; e redução da glicemia e do perfil lipídico, exceto do nível de triglicérides dos hipertensos do G1. Na avaliação da mudança de comportamento frente à atividade física, no final do estudo, o grupo G2 obteve maiores escores (4,00 (3,00 4,00) vs 3,00 (2,00 4,00), e 61% dos hipertensos do grupo G2 passaram para ativos ou muito ativos vs 30,3% do grupo G1 (p<0,05). Houve aumento no controle dos hipertensos no final do estudo, em relação ao início, tanto no grupo G1 (51,5% vs 71,5%) como no G2 (48,1% vs 79,7%) e os hipertensos controlados realizaram mais passos que o grupo não controlado no início e final do estudo. Conclusão O incentivo à prática de atividade física com orientações da enfermagem somada ao uso de um instrumento como o pedômetro mostrou-se eficaz para a mudança de comportamento dos hipertensos estudados.

Introduction Studies have shown the importance of regular physical activity in arterial hypertension. Objectives Assess the effect of activities by using the pedometer, oriented by the nursing team, in the practice of physical activity and the change of hypertensive patients behavior accompanied in Health Family strategy. Method A prospective, experimental study, has been carried out for 10 weeks, in a Basic Health Unit located in São Paulos west side. 263 sedentary hypertensive patients have been studied accompanied in Health Family strategy (70,3% women, 57.4% non-white, 56.4 about 9,6 years). The hypertensive patients classified as inactive by the International Physical Activity Questionnaire (IPAQ) were divided in Group 1- control (G1, n=130) and Group 2- intervention (G2, n=133). The hypertensive patients from both groups were assessed in the beginning and in the end of the study concerning physical activity level, behavior change stage, casual blood pressure measure taken by a nurse and home blood pressure measurement (HBPM), with validated automatic device, and the measurement of steps for 3 days by using the pedometer. Only the G2 hypertensive patients have been provided with guidance and encouragement by the nursing team towards habits change concerning physical activity and pedometer use for 10 consecutive weeks. Results There was statistically significant change (p<0,05), in the hypertensive patients from both groups studied (G1 and G2), in the end of the study in the following variables: casual blood pressure measurement reduction (G1-139,4±19,1/83,1±12,1 vs 130±15,2/79,1±10 e G2-138,7±19/83,6±10,9 vs 128,7±14,8/76,2±10,7 mmHg) and home blood pressure measurement (HBPM) (G1-131,59±18,21/78,12±9,49 vs 126,71±14,74/78,30±10,57 e G2-131,41±17,95/79,35±9,27 vs 126,54±13,51/76,29±8,99 mmHg), except for the diastolic HBPM from the G1 hypertensive patients; an increase in the number of steps, even higher in the G2 hypertensive patients in the end of the study (6331±3202 vs 8108±3103 steps); body mass index and waist circumference reduction, however waist circumference in the men from G2 group was smaller than the men from G1 in the end of the study; blood glucose and lipid profile reduction, except for the triglyceride level from G1 hypertensive patients. Regarding the behavior change assessment towards physical activity, in the end of the study, the hypertensive patients from G2 have obtained higher score (4,00 (3,00 4,00) vs 3,00 (2,00 4,00), and 61% from G2 hypertensive patients have become active or very active vs 30,3% from G1 (p<0,05). There was a control increase in the end of the study of the hypertensive patients from both groups G1 (51,5% vs 71,5%) and G2 (48,1% vs 79,7%), and the controlled hypertensive patients had a higher number of steps than the non-controlled group in the beginning and in the end of the study. Conclusion The encouragement towards the practice of physical activity with the nursing team guidance in addition to the use of a device such as the pedometer have shown effectiveness concerning behavior change in the hypertensive patients studied.
