Abstract in English
This is aimed to describe and analyze the control actions of the communicants of tuberculous people in relation to the physical area, material and human resources, and to the laboratorial net for the investigation of the disease. Data of the following variables were collected: those related to the characteristics of the tuberculous index cases as for sex, age, and clinical form, registered communicants, and those examined as for sex, age, kind of kinship with the index case, types of exams performed and disease incidence. The study population is constituted by 166 communicants registered and examined in the health unity and 7 professionals who develop the actions of the Tuberculosis Control Program. The data collecting tools were: documental analysis, semi-structured interviews and registers of the free observations by means of a field diary. The results indicate that the physical area does not fulfill the regulations of the Health Ministry; all the Health Ministry handbooks for the Tuberculosis Control Program were used. Of the professionals acting in the program, 85% were female and 15%, male; their age range from 32 to 67; the professionals' time from graduation range from 10 to 42 years, and their working time in this activity, from 8 months to 18 years. Of the 112 cases of tuberculosis notified, 77 (68.8 %) were of the pulmonar kind and 35 (31,2 %), of the extra-pulmonar kinds, the male prevailing in 62 cases (80.5 %) over 15 (19.5%) female; 33 (29.5%) were co-infected with TB/HIV. In the communicants of tuberculous people, 263 were identified and registered by the service, as related to 112 index cases. It was verified that, of the 263 registered communicants, 166 (63.1 %) were evaluated as annotations were found in their files; 147 (88.5%) were evaluated by the Tuberculosis Control Program in the Assistance Administration Nucleus 60, 19 (12%) of which, for being under 12, were evaluated in other city health services; the other 97 (36.9%) were either evaluated or not, and this information was not registered in their files, 82 (49.4%) being male and 84 (50.6%), female; as for age, 42 (25.3%) range from 0 to 19, 73 (44%), from 20 to 49, and 44 (22.5%) are 50 or older. Of the diagnostic exams in the communicants, 95.8% performed through thorax X-ray, 12% through mucus bacilloscopy, 7.2 % through tuberculinic test, and 6 % through culture; 3 (1.8 %) developed the disease, completing 2.7 % of the total diagnosed cases. The results also indicate the necessity of revising the Tuberculosis Control Program, emphasizing the surveillance and monitoring of the communicants, developing strategies through systematized protocols for the health professionals to assure the continuity of this assisting service in an integral and human form with emphasis on preventive actions.
