Esta tese apresenta um estudo de avaliação da oferta dos métodos cirúrgicos em Ribeirão Preto que foi implantada a partir de 1999. Para proceder a esta avaliação contextualizamos a questão da escolha de um método de esterilização cirúrgica utilizando como referencial os direitos sexuais e reprodutivos, abordamos o contexto do planejamento familiar no Brasil e da oferta da contracepção no estado de São Paulo e no país. Nesse sentido, utilizamos estudos anteriores já publicados como artigos em revistas científicas ou capítulos de livros. Além disso, o estudo também se contextualiza no referencial da avaliação dos serviços de saúde. Antes da apresentação do estudo há a descrição da proposta de implantação e a realização de um estudo piloto. O estudo de avaliação foi realizado por meio de três componentes: um estudo usando uma amostra de 336 candidatos ao procedimento cirúrgico no período de outubro de 1999 a maio de 2004, um estudo de 230 candidatos que não obtiveram o procedimento, com os quais realizou-se uma entrevista domiciliar usando um questionário semi-estruturado e um estudo qualitativo realizado com 59 profissionais de saúde utilizando quatro sessões de grupos focais de discussão. Teve-se como objetivo do primeiro componente descrever a distribuição anual dos dois tipos de procedimentos e compara-las, descrever o perfil e antecedentes de contracepção dos candidatos à esterilização cirúrgica e verificar a adequação dos candidatos à Lei 9.263 em relação à idade e número de filhos. Ao que concerne ao segundo componente, os objetivos foram caracterizar os indivíduos que não obtiveram o procedimento de contracepção cirúrgica e a identificação de fatores relacionados à não obtenção da cirurgia. Em relação aos grupos focais procurou-se identificar como os profissionais de saúde avaliavam suas atividades no planejamento familiar e identificar suas principais dificuldades. Nos resultados observou-se que com o tempo houve um aumento de 4,7% de laqueaduras e uma diminuição equivalente de vasectomias; a maioria dos candidatos era do sexo feminino (71,5%), casadas (90%) e 53,3% não haviam concluído o ensino fundamental, sendo católicos (66,9%), metade morava em casa própria, tinham em média 3,19 filhos, renda familiar média mensal de R$ 613,11, 94,3% já haviam utilizado algum método anticoncepcional alguma vez na vida e 90,5% usavam algum contraceptivo quando procuraram o serviço. Os motivos para esterilizar-se mais citados foram o econômico (29,3%), satisfação com o número de filhos (22,6%) e ter muito filhos (19,5%). Não foi encontrado nenhum indivíduo esterilizado em desacordo com a lei em relação à idade ou número de filhos. Dos 336 candidatos à esterilização cerca de 11,6% (39) não obtiveram o procedimento. O tempo médio de aconselhamento para obtenção do procedimento foi de cerca de 8 meses. Realizada análise por regressão logística de 267 casos de casais que obtiveram a cirurgia verificou-se que há maior chance de obter vasectomia para homens acima de 39 anos (OR 4,9), ser casado (OR 41,1) e menor número de gestações (OR 2,3). Dos 230 indivíduos que não obtiveram o procedimento 21,3% eram homens e 78,7% mulheres, sendo a maioria casada, branca com 4 anos de escolaridade, e católicos. Tinham em média 3,26 filhos vivos e uma renda familiar mensal de 720,55 reais. Perguntados por que não haviam realizado a cirurgia 10% referiram estar aguardando serem chamados e 9,6% já haviam se submetido à cirurgia em outro serviço; 1,7% referiu ter engravidado; 6% referiram problemas burocráticos e 8,7% problemas de saúde. Um contingente de 52,1% aparentemente desistiu da cirurgia já que entre estes 18,2% referiram falta de tempo, 14,8% ficaram com medo, 11,7% disseram ter pensado melhor, 2,6% disseram ser jovem e 4,8% referiram que o/a parceiro/a não permitiu. Entre todos, 94% disseram terem sido bem aconselhados. Estudados os fatores entre os que obtiveram o procedimento e os que não obtiveram constataram se diferenças significantes em não obter o procedimento para mulheres, indivíduos acima de 39 anos, evangélicos, renda per capta mais elevada e ser solteira. Os grupos focais resultaram que os profissionais de saúde se realizam e se empenham em seu trabalho, considerando-o efetivo e resolutivo, mas enfrentam problemas de infraestrutura na rede básica de saúde, tais como falta de local para atendimento, agenda superlotada, falta flutuante de contraceptivos, falta de integração com os médicos no trabalho em equipe. O estudo conclui que há pontos positivos e que a oferta não atende apenas a cerca de 10% dos candidatos. Há espaço para melhorias em relação ao tempo de espera, cobertura mais eficaz de métodos contraceptivos, expansão da dupla proteção e integrações de ações.

This thesis presents an assessment of the provision of the surgical sterilization implemented in Ribeirão Preto since 1999. To perform this assessment the issue of choosing one method of surgical sterilization was considered using as conceptual framework the sexual and reproductive rights, we approached the context of family planning in Brazil and the provision of contraception in São Paulo State and in the country. For this, we used previous studies that were published as papers in scientific journals or as chapter in books. Furthermore, we also used as context of this analysis the conceptual framework of health services evaluation. Prior presenting the study there is the description of the implementation proposal and the pilot study. The assessment was conducted through three components: a study using a sample of 336 candidates to the surgical sterilization who were enrolled in the program from October 1999 to May 2004, a study of 230 candidates who had not obtained the procedure and to whom a semi structured questionnaire was administered in their household and a qualitative study carried out with 59 health professionals using four sessions of focus group discussion. The objectives of the first component were to describe the annual distribution of the two types of procedure and compare them, to describe the profile and former contraceptive use of the candidates to surgical sterilization and to verify the adequacy of the candidates to the Federal Act 9.263 regarding age and number of children. Concerning the second component the objectives were characterizes the individuals who had not obtained the procedure of surgical contraception and to identify the factors related to its non-attainment. Regarding the focus group discussion it was aimed to identify how the health professionals assessed their activities in the family planning and to identify their main difficulties. In the results it was observed that over time there was an increase of 4.7% of tubal ligations over vasectomies; most of candidates were female (71.5%), married (90%) and 53,3% had not finished grammar school, being Catholics (66.9%), one half owned the house they lived in, had an average of 3.19 children, the mean of the monthly family income was R$ 613,11, 94.3% had ever used any contraceptive and 90.5% used any contraceptive when enrolled in the program. The most presented reasons to search for sterilization were the financial (29.3%), satisfaction with the number of children (22,6%), and having too many children (19,5%). No individuals were found sterilized in disagreement with the law in attention to number of children and age. About 11,6% of 336 candidates have not obtained the procedure. The average time of obtainment and counseling was around 8 months. The logistic regression analysis of the 267 cases of couples who obtained the surgery resulted that there is more chance to have a vasectomy for men older than 39 years old (OR 4.9), married (OR 4.1) and few number of pregnancies (OR 2.3). Out of 230 people who had not obtained the surgery 21.3% were male and 78.7% were females, most married, white, with four years of schooling and Catholics. They had a mean of 3.26 living children and a monthly family income of R$ 720,55. When asked why had not obtained the surgery 10% referred they were awaiting to be called, 9.6% had dropped out the program and obtained the surgery elsewhere, 1.7% became pregnant, 6% reported bureaucratic problems and 8.7% reported health problem. A total of 52.1% apparently gave up to undergo the surgery since they reported that had no time for this (18.2%), were scared of the procedure (14.8%), had second thoughts about it (11.7%), they said were too young (2.6%) and reported the partner had not allowed them (4.8%). Almost all of them (945) reported they were well counseled by the program. Among the factors which were identified between those had not obtained the surgery being woman, above 39 years old, Evangelic, having higher income and being single can decrease the chance to get the surgery. The focus group discussion resulted that health professionals fulfill themselves and make an important effort to carry out the activities but they face many infrastructure problems in the local health services, such as lack of rooms to see the clients, full booking schedule, interruption of contraceptive provision and lack of the health team integration. The study concluded that there are many positive points in the program but the provision fails to attend about 10% of the candidates. There are room for improvements regardind the awaiting time, better coverage of contraceptives, dissemination of double protection and integration of actions.
