Objetivos. Documentar a magnitude da mortalidade por câncer em São Paulo, e identificar quais tipos (localização anatômica) foram responsáveis por incremento, redução ou tendência estacionária no período recente. Efetuar a distribuição espacial dos dados de mortalidade por câncer e de condições socioeconômicas na cidade, visando explorar padrões geográficos de associação entre estas dimensões. Métodos. Dados oficiais sobre óbitos e população permitiram estimar coeficientes de mortalidade ajustados por sexo e faixa etária por tipos de câncer. O estudo de séries temporais contemplou o período 1980-2002; a análise espacial discriminou informações (1995-2002) para os 96 distritos da cidade. Dados censitários e outras informações sobre população permitiram construir medidas de desenvolvimento social. As variáveis do estudo foram objeto de estimação de tendências e de associação espacial. Resultados. Câncer de traquéia, brônquios e pulmões (13,58 óbitos por 100.000 habitantes), cólon e reto (10,48), estômago (9,74), mama (18,92, sexo feminino) e próstata (13,54, sexo masculino) foram os tipos com maior mortalidade no período 2000-2002. Tendências crescentes mais elevadas foram observadas para câncer de cólon e reto (16,43‰), pâncreas (16,40‰), rim (14,86‰), próstata (19,47‰, para homens) e traquéia, brônquios e pulmões (13,20‰, para mulheres). Câncer de boca e faringe, esôfago, estômago, laringe, colo de útero e útero (sem outras especificações) apresentaram taxas mais elevadas de mortalidade nos distritos de pior condição social. Conclusões. Diferenciais de risco e de uso dos serviços de saúde foram discutidos quanto às implicações para futuras pesquisas e intervenções dirigidas à modificação do perfil epidemiológico do câncer em nosso meio.

Objectives. To document the magnitude of cancer mortality in São Paulo and to identify which types (by anatomic localization) underwent increasing, decreasing or stationary trend of deaths during recent years. To perform spatial data analysis of cancer mortality rates and socioeconomic indices in the city, aiming to explore geographic patterns of association between these dimensions. Methods. Official information of deaths and population allowed the estimation of gender- and age-adjusted mortality rates for different types of cancer. Time-series analyses referred to the period 1980-2002; spatial data analyses gathered information (1995-2002) for each of 96 districts of the city. Measurements of socioeconomic status used census information and other sources of population data. Study variables were subjected to trend estimation and spatial association. Results. Cancer in the trachea, bronchium or lung (13.58 deaths per 100,000 habitants), colon or rectum (10.48), stomach (9.74), breast (18.92, for females) and prostate (13.54, for males) were the types with higher mortality during 2000-2002. A steeper increasing trend was observed for colorectal cancer (16.43‰), pancreas (16.40‰), kidney (14.86‰), prostate (19.47‰, for males) and trachea, bronchium or lung (13.20‰, for females). Cancer of mouth or pharynx, esophagus, stomach, larynx, cervix and uterus (not otherwise specified) presented higher death rates in deprived areas of the city. Conclusions. Differentials of risk and use of health services were discussed as to implications for future research and interventions aimed at modifying the epidemiologic profile of cancer in our midst.
